Your Right to a Good Faith Estimate
Great Minds Psychiatry & Wellness PLLC
If you are paying for your care yourself, or choosing not to use insurance, you have the right to know what it will cost before you are seen.
Effective Date: October 25, 2026
Your right to a Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
How this works at Great Minds
If you are paying for your care yourself, or choosing not to use insurance, we will give you a written Good Faith Estimate:
- When you schedule. At least 1 business day before your appointment if it is scheduled 3 to 9 business days ahead, and at least 3 business days before it if it is scheduled 10 or more business days ahead.
- When you ask. Within 3 business days of your request, even if you have not scheduled anything yet.
Current self-pay rates
| Service | Self-pay rate |
|---|---|
| Initial psychiatric evaluation | $250 |
| Follow-up visit (medication management) | $150 |
| QbCheck computerized ADHD screening (not covered by insurance) | $150 |
Because psychiatric care often involves ongoing follow-up visits, your estimate may cover a series of expected visits over a period of up to 12 months. If your expected care changes, we will give you an updated estimate.
A Good Faith Estimate is not a contract and does not require you to receive the services listed. Your actual care may differ based on your clinical needs.
How to request one
To request a Good Faith Estimate, call (804) 295-4344 or email info@greatmindswellness.com. You can ask at any time, including before you schedule.
