Documentation and forms
Great Minds requires a minimum of six months of continuous treatment engagement before documentation or forms, such as letters or paperwork requiring provider certification, can be completed for a patient.

Do you accept insurance?
Yes. Accepted plans depend on your state: see the lists on this page. Patients are encouraged to verify eligibility and benefits with their plan prior to their appointment.
Do you accept Virginia Medicaid?
Yes. Virginia Medicaid is accepted, and eligibility is verified the same day.
What if I don't have insurance?
Self-pay rates are available: $250 for an initial evaluation and $150 per follow-up visit.
Is QbCheck ADHD testing covered by insurance?
No. QbCheck is $150 self-pay and is not covered by insurance.
Is GeneSight covered by insurance?
GeneSight is billed by GeneSight. Please contact GeneSight Billing & Insurance Support at (844) 697-4239 to verify your coverage before requesting the test.
Do you provide documentation or forms?
