Notice of Privacy Practices
Great Minds Psychiatry & Wellness PLLC
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective Date: October 25, 2026
Our commitment to your privacy
Great Minds Psychiatry & Wellness PLLC (“Great Minds,” “we,” “us,” or “our”) is required by law to maintain the privacy of your protected health information (“health information”), to give you this notice of our legal duties and privacy practices, and to notify you if a breach of your unsecured health information occurs. We must follow the terms of the notice currently in effect.
Health information includes information that identifies you and relates to your past, present, or future physical or mental health, the care you receive, or payment for that care.
How we may use and share your health information
For treatment
We use and share your health information to provide, coordinate, and manage your care. For example, we may share information with a pharmacy to fill a prescription, with a laboratory that processes tests we order, or with another provider involved in your care.
For payment
We use and share your health information to bill and get paid for the services we provide. For example, we may give your health plan information about your diagnosis and the services you received so the plan will pay for them.
For health care operations
We use and share your health information to run our practice, improve care, and contact you when necessary. For example, we may use information to review the quality of our services, to train staff, or for business planning and administration.
Appointment reminders and service information
We may contact you to remind you of an appointment, or to tell you about treatment options or health-related services that may interest you. We may do this by text message, email, telephone, or through our secure patient portal, using the contact details you give us. You may ask us to use a particular method or to stop a particular method at any time.
Business associates
We work with outside companies that perform services for us, such as our electronic health record and scheduling system, billing services, and secure communication tools. We share health information with them only under a written agreement that requires them to protect it.
Other ways we may use or share your health information
We may use or share your health information without your written authorization when permitted or required by law, including:
- When required by law. When federal, state, or local law requires it.
- Public health activities. For example, to prevent or control disease, or to report suspected abuse or neglect of a child or vulnerable adult as required by law.
- Health oversight. To agencies that oversee the health care system, such as for audits, investigations, inspections, and licensure.
- Legal proceedings. In response to a court order, and in some cases in response to a subpoena or other lawful process.
- Law enforcement. For certain law enforcement purposes, as permitted by law.
- To prevent a serious threat to health or safety. When necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public. We follow the law of the state where your care is provided, which may require or permit us to warn or protect an identified person.
- Coroners, medical examiners, and funeral directors. As needed for them to carry out their duties.
- Organ and tissue donation. To organizations that handle organ, eye, or tissue donation.
- Research. Under certain conditions and with required privacy protections.
- Specialized government functions. For example, for military, national security, or protective services, as permitted by law.
- Workers’ compensation. As authorized by workers’ compensation laws.
- Prescription monitoring programs. We may check or report to state prescription drug monitoring programs as required by law when prescribing certain medications.
People involved in your care
Unless you object, we may share relevant health information with a family member, friend, or other person you identify who is involved in your care or in paying for it. If you are unable to agree or object, for example in an emergency, we may share information if we believe it is in your best interest.
Uses that require your written authorization
We will not use or share your health information for the following without your written authorization:
- Psychotherapy notes. Most uses and disclosures of psychotherapy notes (a clinician’s separate notes analyzing the contents of a counseling session) require your authorization, except in limited situations allowed by law.
- Marketing. We will not use your information for marketing without your authorization.
- Sale of health information. We will never sell your health information.
Other uses and disclosures not described in this notice will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights
You have the right to:
- Get a copy of your health records. You can ask to see or get an electronic or paper copy of your health information. We will usually respond within 30 days. We may charge a reasonable, cost-based fee.
- Ask us to correct your records. You can ask us to correct health information you believe is incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
- Request confidential communications. You can ask us to contact you in a specific way (for example, a particular phone number) or at a different address. We will say yes to all reasonable requests.
- Ask us to limit what we use or share. You can ask us not to use or share certain information for treatment, payment, or operations. We are not required to agree, except in one case: if you pay for a service or item out of pocket in full, you can ask us not to share that information with your health plan, and we will say yes unless a law requires us to share it.
- Get a list of those with whom we have shared information. You can ask for a list (accounting) of the times we have shared your health information for six years prior to the date you ask, who we shared it with, and why. This list will not include disclosures for treatment, payment, or operations, and certain others. We will provide one list a year for free.
- Get a copy of this notice. You can ask for a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. If you have given someone medical power of attorney, or someone is your legal guardian, that person can exercise your rights and make choices about your health information.
- Be notified of a breach. We will notify you if a breach occurs that may have compromised the privacy or security of your information.
- File a complaint. See “Questions and complaints” below.
Minors
When we provide care to a minor, a parent or legal guardian is generally the minor’s personal representative and may exercise the minor’s rights under this notice. Some state laws give minors the right to consent to certain mental health services and to control access to related information. We follow the laws of the state where care is provided.
State law
HIPAA sets a minimum standard. Where the law of the state in which your care is provided gives you greater protection, we follow that state law. This can affect mental health records, when a clinician may or must disclose information to prevent harm, and what a parent may access for a minor. Great Minds provides care to patients located in Virginia, Washington, D.C., Maryland, Washington State, Colorado, Connecticut, Arizona, and Massachusetts.
Changes to this notice
We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our website.
